Abstract :

Introduction: The infectious endocarditis is a systemic disease with polymorphic presentation. We report the observation of a patient with transient amaurosis of the left eye reveals an infectious endocarditis. Observation: Mr. R. A is 45 years old, who consults for a sharp monocular decline of visual acuity. The examination of the left eye finds a visual acuteness: + PL. At the bottom of eye, there is a pale retinal appearance with a cherry red macula compatible with retinal ischemia. The retinal angiography showed a filling delay in the territory of the upper and lower temporal artery, related embolic origin. The examination of the right eye was normal. The clinical examination found a febrile patient with a rolling mitral and aortic diastolic murmur, with no evidence of heart failure or endocarditis skin lesions. The echocardiography objective tight mitral stenosis, massive aortic insufficiency with a mobile vegetation 12 mm on the right coronary cusp of the aortic valve, and another of 7 mm on the posterior cusp. The infectious balance sheet was positive: CRP 86 mg/l and leukocytosis 20,000 /mm3. Blood cultures were negative. Rheumatoid factor was positive. Abdominal ultrasound and cerebral scanner did not objectify other embolic lesions. The diagnosis of the infectious endocarditis was retained and the patient put under bi-antibiotic treatment (C3G + Gentamicine) with a good clinical and biological evolution. Conclusion: The occlusion of the central artery of the retina is a rare and serious complication of the infectious endocarditis. The prognosis depends on the degree of arterial recanalization.
